In an effort to include information that assists students and practitioners to use best practices, we have added a substantial amount of information and included additional resources for further investigation. Our format integrates current information and research, principles of sound clinical practice and practical experience. Our intention is to outline the procedures and rationale of case management and clinical documentation to give readers some background as well as information regarding current practice.
